The Care Manager plays a central role in building meaningful partnerships with children and families while coordinating care with system partners. This position serves as a key facilitator of the Child and Family Team (CFT) process, empowering families through strength-based, individualized planning. The Care Manager works collaboratively to ensure that all support and interventions align with the youth’s goals, ensuring continuity of care and access to the least restrictive, most effective services. The ideal candidate is passionate about working with youth and families, understands the complexities of behavioral health needs, and is committed to the values of equity, empowerment, and positive outcomes. This is a community-based position requiring regular travel to family homes, service provider locations, and community-based settings. This position may involve exposure to emotionally challenging situations and requires emotional resilience, cultural sensitivity, and a strong sense of professional boundaries.
